I believe that the school is not in compliance with MSER, Chapter 101. I am basing this belief on ___________ [ describe your complaint(s) in one or more of the areas of identification, programming, placement or FAPE; and list any evidence that supports this ]. (If appropriate you might include copies of documentation that supports this; i.e.: current IEP, Quarterly Reports, work samples, etc.)

I understand under MSER, Chapter 101, Section 12.11G, that I will receive a response and any corrective action to be taken to resolve this complaint within 30 days of your receipt of this written request.
