Chapter 9: Getting Older

Timeline for Transition Planning

Ages 18-20

Members of team shift from school-based to community and agency based. Student needs to either receive a regular high school diploma or “age out” of special education.

Age 17

Inform students of special education rights (age of majority).

Age 16

Determine Transition Services. Link with appropriate adult agencies.

Age 14

Determine courses of study based on student's long term goals and needs.

Throughout a child's life -

Teach and encourage decision making skills, independent living skills, and self-advocacy skills.
